BarrierNotes Academy — evidence-request worksheet Keep functional-safety handover questions open Learning reference: BN-AW-FS-01 · version 1.0.0 Fictional awareness practice. This record does not certify competence or approve equipment, a safety function or operation. Use fictional facts and role names. Keep confidential records in your responsible project process. Fictional dossier: vessel V-7 Hazard note H-3 revision 1: Unwanted heating may lead to a temperature rise, loss of containment and exposure. Stored heat or an ongoing reaction may continue after the heater is isolated. Draft function DF-2: 'On a high-high vessel-temperature condition, isolate the heating supply.' The trigger threshold and required safe outcome are not yet justified. Requirements draft R-0: Says 'SIL 2, response within 2 seconds — same as the previous vessel'. No risk-allocation or process-response evidence is attached. Bench note B-6: Records that a controller output relay changed state during a bench stimulus. It does not describe installed sensor/final-element response, the vessel's process response or comparison with approved requirements. Draft handover: 'Controller tested. Shutdown is safe. Start-up ready.' 1. Received record and configuration ____________________________________________________________ 2. Narrow claim supported ____________________________________________________________ 3. Unresolved requirement or outcome evidence ____________________________________________________________ 4. Evidence provider and responsible review ____________________________________________________________ 5. Review trigger and handover status ____________________________________________________________ Written practice Activity 1: write a bounded candidate statement Rewrite the draft handover in two sentences: accurately state the proposed trigger/action, then state why the dossier does not establish a safe outcome or start-up readiness. Do not choose a threshold or another protective action. ____________________________________________________________ Activity 2: prepare three evidence requests Write one request about the process outcome, one about the copied response time/SIL, and one about the bench note's scope. Give each request a responsible review route. Use role names only. ____________________________________________________________ Activity 3: preserve the change trail New fictional information: after the bench check, the designer replaces the output relay with a different final-element arrangement. Write the updated handover and the next question. You have no test evidence for the new arrangement. ____________________________________________________________ Review your own work against the public examples.
